Using the coding techniques you have learned, carefully read through the case study and determine the most accurate diagnosis code(s). Remember, check the chapter-specific, subchapter-specific, and category-specific notations within the Tabular List. Then, determine the most accurate procedure code(s) along with any modifier(s), if appropriate. BROADMORE IMAGING CENTER 234 DOPPLER ROAD • ANYTOWN, FL 32711 • 407-555-5977 PATIENT: KLEMPTER, BRANDY ACCOUNT/EHR #: KLEMBR001 DATE: 06/17/25 INDICATION: Stenosis brachiocephalic artery Procedure: Arch aortogram, right brachiocephalic arteriogram, angioplasty, and stent right brachiocephalic PROCEDURE: The patient was informed of risks associated with the procedure, including stroke, and agreed to the procedure. The patient’s right groin was sterilely prepped and draped in the usual fashion. Via retrograde right femoral arterial approach, 4 Fr vascular sheath was inserted. 5 Fr pigtail catheter was then advanced into the ascending aorta. An arch aortogram was then performed in the left anterior oblique projection. The arch aortogram demonstrated a high-grade stenosis present at the origin of the right brachiocephalic artery. A Simmons II catheter was inserted and the catheter was reformed in the ascending aorta. The right brachiocephalic was selectively catheterized. Supra Core guide was advanced into the right subclavian artery, and the catheter was positioned within the subclavian artery. The guide wire was advanced into the axillary artery. The catheter was then removed, and a 7 Fr shuttle sheath was advanced to the origin of the right brachiocephalic artery. 8 mm × 2 cm long angioplasty balloon was then inserted. The patient received 3,000 units of heparin intravenously. The area of stenosis was then balloon-dilated. A repeat arteriogram was obtained demonstrating marked residual stenosis. A 12 mm × 30 mm long LUMINEXX stent was then inserted and deployed in the usual fashion across the area of stenosis. A 10 mm × 2 cm long angioplasty balloon was then inserted, and the stent was further balloon-dilated to a 10-mm diameter. A repeat arteriogram was obtained demonstrating good angiographic result with no significant residual stenosis identified. The catheter and sheath were then removed and hemostasis was obtained with Perclose suture device. IMPRESSION: High-grade stenosis origin of the right greater cephalic artery. This was successfully angioplastied and stented to a 10-mm diameter as described above. Keith Robbins, MD Be sure to list the codes, one code per box, in the correct sequence, from top to bottom, and in the proper row. Capitalization, punctuation, and spacing can impact whether or not your answer is correct. Follow coding best practices. Please list modifiers with the code they relate to as necessary. Include a hyphen in between the code and the modifier. Example Procedure: 43846-74 or for Anesthesia: 00797-P2.
