You will identify the ICD-10 diagnosis, determine which diagnosis is primary and which is secondary, identify the main terms, and use SelectCoder to code in ICD-10. Use this patient record. This is an outpatient record and will look different from the inpatient record you will use in Milestones Two and Three and the final project. This is a good time to iron out any specific issues you are having and to contact your instructor with questions. There is a great deal of information in the patient record. For this first milestone, you will review the entire record and then follow the steps below to practice assigning the proper ICD-10 codes for the highlighted “Problems” section of the record. Specifically, the following critical elements must be addressed: ICD-10: In this section, you will review the patient medical record and apply the appropriate ICD-10 coding to the diagnosis. You will also be asked to provide your rationale for how you arrived at a particular code. Remember that there will be more than one code if there is more than one presenting problem. Describe your process for determining all necessary codes. Review the chart notes and determine a diagnosis. Explain how you arrived at your determination.Determine the primary and secondary diagnosis.
