Client had sore both top shoulders, pain shooting on left side neck. Asked for back and neck worked and both hips. Client retired. Had an accident at work. Tore all four tendons in left shoulder. Had surgery over a years ago to reattached. Some years ago to reattached. Refused medication. Using contrast therapy on hands and shoulders. Please provide soap notes:
