Return to clinic for follow-up in 3 months. 4/11/xx: Client reports increased fatigue since last visit and now also having increased thirst, hunger, and urination. Laboratory tests ordered. 4/18/xx: Reviewed laboratory results with client. New provider prescription. Provided client education about new prescription.
