Emmet Steel, 64 years old divorced forklift driver, has a long history of COPD, alcoholism, and osteoarthritis. He was diagnosed with inoperable stage IV non-small-cell lung cancer 3 months ago and has had 2 cycles of chemotherapy. Mr Steel was admitted to the hospital 16 days ago with difficulty breathing, chronic cough, and severe SOB with any exertion. He continues to require hospitalization for fever and pancytopenia. Admission CXR revealed bilateral pleural effusions with greater involvement on the right, and an empyema located in the right middle lobe. Two chest tubes were placed. While he has been in the hospital Mr Steel has had no visitors other than his sister who is a single parent raising 5 children under the age of 14. Mr Steel remains a full code and continues to deny any conversation with the physician about disease progression. The hospital admission form states Mr. Steel does not have advance directives, power of attorney, or a healthcare representative. Efforts by the critical care nurses to discuss these issues have been met with silence. Today his sister conveys to the critical care nurses that she is very upset because the physician returned a call regarding her brother’s prognosis and stated he was going to start TPN before the start of the third cycle of chemo. The sister asks the nurse to change her brother’s code status to no code. She states she is very concerned about the increasing pain and confusion of her brother and there is no need for another round of chemo.
