Need help developing a Nurses Care Plan for Fall Risk for a practice case study. Need to develope the care plan using SMART outcomes. Patients background Abigail Cantonelli, age 80, injured her left knee and wrist when she fell on an icy sidewalk. She has been on your orthopedic and neurologic unit for several days. She has a history of cardiomyopathy, for which she receives furosemide. Her vital signs are stable and she rates her pain as aching, intermittent, and a 2 on a scale of 1 to 10 (10 = worst pain). Because Mrs. Cantonelli has an increased risk of falling, she has been prescribed physical therapy and cane-walking instructions before discharge. Although the physical therapy staff has already initiated the cane-walking instructions, you will need to ambulate Mrs. Cantonelli with her cane during your shift. While you are ambulating down the hall, she says, “Oh, dear! I feel dizzy.” She begins to lose her balance and fall toward you. Here is a care plan I did, please let me know if it is done correctly and what I can add to it. Patient Name: Abigail Cantonelli Dx/Condition: Risk for Falls related to impaired physical mobility and dizziness as evidenced by recent fall and report of dizziness. Reevaluation Date: 01/28/2025 Fall risk Goals: The patient won’t experience any falls. The nurse will demonstrate to the patient safe cane-walking techniques with minimal assistance by the end of the week. The patient will acknowledge and comply with fall prevention measures. Nurses will educate on cane use and safety measures to prevent further falls without assistance by the end of the week. Interventions Perform a falls assessment, per facility protocol, that evaluates the history of previous falls, patient mobility, cognition, and mental status. The nurse will assess the patient’s current condition and risk factors for falls. Consider her age, recent fall, history of cardiomyopathy, and medication use (furosemide) which can cause dizziness and orthostatic hypotension. Review the patient’s medications with the patient and family (as appropriate) to identify possible adverse effects that may cause falls. The nurse will educate the patient and family in the next three days that the medication use (furosemide) the pt is on can cause dizziness and orthostatic hypotension. Implement falls precautions, as appropriate. Ensure adequate lighting and appropriate nonskid footwear while the patient is ambulating. Educate the patient and family (as appropriate) about measures they can take at home to alter the environment to promote safety and prevent falls. The nurse will educate Mrs. Cantonelli on the importance of using her cane correctly and follow up with physical therapy by the end of the week. Involve the patient and family (when appropriate) in a plan to stay safe. The nurse will explain to the family by the end of the week that assisting the patient with ambulation will help to prevent future falls and encourage the patient to use the cane in a safe manner and notice signs of dizziness. Collaborate with a physical therapist, as needed. The nurse will notify the patient to follow up with the physical therapist within the next day after discharge.
