You work in as a nurse on a geriatrics ward. Mrs Osei was admitted three weeks ago with a UTI that caused her to be very tired and confused, resulting in a fall where she sprained her wrist. She has now recovered sufficiently to be ready to be discharged. Her nephew Eric, who is her main family contact, is insistent that Mrs Osei can no longer cope living alone and should be discharged into a care home close to where he lives. However, Mrs Osei does not have sufficient savings to afford the fees and would need to be funded by the local authority. The local authority has proposed an at-home care package rather than discharge into a care home, as this is more cost-effective. You have been invited to the meeting where health and social care professionals involved in Mrs Oseis care at the hospital will advise the local authority about Mrs Oseis needs and what would be an appropriate discharge process. At the meeting:Mrs Osei is embarrassed by the amount of support she needs. She makes several statements that you think are untrue, such as that she doesnt need any help going to the toilet and that she is able to eat meals without assistance. Eric, the nephew, is angry that Mrs Osei has said that she doesnt mind going home as long as it means she can leave hospital. He is worried about her living alone and having another, more serious fall. It is difficult for him to provide her with extra support, as he lives 45 minutes away and works full-time. He thinks Mrs Osei should be pushing to be accepted into the care home. The social workers from the local authority seem to be placing a lot of emphasis on the physiotherapists assessment that Mrs Osei can walk steadily without needing a walking frame and so wouldnt be at risk of falls at home. You know that the physiotherapist has only visited Mrs Osei for fifteen minutes and, based on your observations, think that Mrs Osei cannot always walk steadily. The outcome of the meeting is that Mrs Osei will be discharged back to her own home with a care package of four carer visits per day for two weeks. The care package will then be reassessed. Eric is upset with this decision and, after the other health and social care professionals have left, accuses you of not doing enough to change their minds. In the written assessment, you should 1.Be able to define assertion and aggression and non-assertive behaviour2.Be able to explain the distinction between assertion and aggression and non-assertive behaviour using examples from the case study. For example, which of the types of behaviour above do you think the individuals including Mrs Osei might be demonstrating? 3.Be able to explain how effective and assertive communication can improve care in-certain situations. Your answer must refer to the case study.
